Follow-up visit after a hospital or rehab stay.
Service Description
Best scheduled within 2 days BEFORE a hospital or rehab discharge. We review your discharge papers and medications, answer questions, and help you watch for warning signs, so you don’t end up back in the ER or hospital. We also help you understand how your new meds fit into your medication plans, and we sign you up for SELF monitoring service so that we can keep an eye on your vital signs.
Cancellation Policy
House Call provides virtual visits by phone and video for seniors and adults with chronic health conditions. To cancel or reschedule, please contact us at least 24 hours before your visit. This allows us to offer the appointment time to another patient who needs care. If you miss your appointment or cancel with less than 24 hours’ notice, a late cancellation / no-show fee may apply and your visit may need to be rescheduled. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Do not use online booking for emergencies.
