Service Description

A yearly telehealth visit to review your medical history, medications, risks and health goals. We check on chronic conditions, update your records, and create a simple prevention plan to help you stay well at home.


Cancellation Policy

House Call provides virtual visits by phone and video for seniors and adults with chronic health conditions. To cancel or reschedule, please contact us at least 24 hours before your visit. This allows us to offer the appointment time to another patient who needs care. If you miss your appointment or cancel with less than 24 hours’ notice, a late cancellation / no-show fee may apply and your visit may need to be rescheduled. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Do not use online booking for emergencies.
